Click here to complete a short online form. It takes less than 5 minutes.

You'll need your Federal Employer Identification Number to complete the form. If you don't know it, you can find it from your accounting, billing or human resources colleagues.

What does it cost?

Nothing. Being part of The Molina Foundation Network is free for organizations who do tremendous work, every single day, to help children, families and individuals in need.

Who can join?

The Molina Foundation is building a growing community of 501(c)3 nonprofit organizations, clinics, medical/dental providers, Title 1 schools, government agencies, faith-based organizations, tribal agencies or other types of programs serving children, individuals (of all ages), and families at-risk and in need.

What happens after we join?

After you complete the form, you will be joined into the Foundation’s Network. Our staff reviews every new program and if we need more information, or something is incomplete, we will reach out within 2-3 days.

You’ll start to receive emails with:

  • Tips you can share with parents to help create word-rich and healthy homes
  • Links to free, downloadable resources
  • Opportunities to apply for grants of new children’s books
  • Updates about the Foundation’s programs
How soon can we apply for books?

Groups in our Network are the first to know about opportunities to apply for new books and other resources from the Foundation. We announce these opportunities via email.

We know many programs are in desperate need of resources – such as books and other educational and health resources. We work every day to find donations of books and resources to share with your program – and we share these opportunities as fast as we can.
